This way, we can build targeted networks of resources that meet the unique needs of every community we serve.

The Community Needs Map Composite Index represents a grouping of four social need indices that track key social risk factors down to the county level. These four indices describe needs relating to socioeconomic status, healthcare access, literacy, social isolation, rural vulnerability and more.
